Participant Contact Information Page

Once a participant selects a category to register for, they complete the Participant Contact Information page, the registration form. 

The following fields are listed by default on the Participant Information Page: First/Last Name, Address, Phone Number, Email Address, Birthdate, Gender, Emergency Contact Name/Number, Team Name (optional).

Custom Questions and Online Waivers will display on this page. You can collect additional information by creating a Custom Question.
